Yes u may fill in the details of ur school. and if there are more than 1 fields, then u may fill up the details of school in 1 and college in other.

not completely that way. Actually 1 copy of ur eval will be sent to the board. However, it is advisable that u send another photocopy of the eval report to the board again, when u apply for the NTS.

They need all the copies of Mark sheets, Degree certificate right from the 10th grade thru MBA. You can get them notarized from any US Notary. Typically you can find them in your Bank and most of time they do it free of cost for customers. How ever depending upon the State, you are expected to submit all your Degrees with Marks in sealed cover directly sent  by your university.  Or you can get the sealed covers from university and mail them to the board. I would suggest you better start working on them as well. as these are must and time consuming unless you are lucky.

10th would do. Small suggestion, do not worry too much on filling the FACS application accurately. You can write a covering letter and enter all your educational qualifications (Marks not required) in a Chronologically order and request them for what purpose you need the evaluation and which state. 

Other best option email/call them with your questions. They are very prompt (FACS).

That is correct, the Original will be sent to the State Board. You will be given a copy. First step in CPA Registration is that one has to get registered with State. The State in turn recommends you to NASBA for CPA exam etc. NASBA will not directly work with you until the time you are an eligible candidate with a  State.

First thing first:

1.      Identify your state.

2.      Submit for Evaluation report (It takes about min of 30 days)

3.      Simultaneously get the Marks and Degrees in a sealed cover from the Indian Universities (Only Degrees are required) .

IN US usually, Bank does this service free of charge. If they ask for money goto another Bank. If you are working you can also find the notaries at your work place. Ask your HR.
