Looks like I don't need to fill out Form 2. Turns out it is for those who completed undergrad studies outside US or completed CPA exam parts in another state.

hey guys, another question.

On Form 4, it asks to list out your CPA examination record. Are you supposed to list all, even the ones you failed. The reason I ask is because they 4 rows to input "date and grade". So each column (Section) has 4 rows. Thanks.
