You can actually call NASBA and ask to speak to the NY Co-ordinator.  I have contacted 2-3 different co-ordinators ( various states) and they are quite diligent in responding.

anmore, you don't complete Form 2 unless you studied abroad or passed the CPA exam in another state.

The top of Form 2 says "Complete this form only if you: (a) completed all or part of your undergraduate or graduate studies claimed for licensure outside the United States; or
(b) completed all parts of the Uniform CPA Examination in another state."

I know, it's confusing.  I was muddling through this process around this time last year.

I just submitted Form 1 (which you don't have to because you already did back in 1989) and the Initial Application

I also has all my education institutions send in the Registered Program Form

even though only one institution was actually registered with NY state as a qualifying program.

I was approved 3 weeks after sending in the Initial Application, even though they say it takes 6-8 weeks!  That put more pressure on me because your NTS is valid for only 6 months after approval date, and I got approved early in April, in the middle of tax season.

You probably will have to submit either the Initial Application

once again - thanks KJ! I have talked to two people at NYS Div of Prof Lic and guess what? Two different answers! They claim they need my transcripts again (purged). They also claim they need Form 1 again. I should be used to NYS at this point, but they are treating this like a state secret. I will follow your links and contact NASBA.

Oh, and I am an initial candidate again - because I never took the computerized exam before. You know, I used the stylus and stone tablet, lol.

At least, despite the fiscal crisis, I do not have to pay an application fee again! That was a shocker.

No prob.  Yeah, that's NY state for you!  No harm sending in transcripts again, but if you don't have the 150 hours, don't send in Form 1 again (especially since the fee is now $377!).  If you send in Form 1 again, they might think you're not eligible for the pre-8/1/09 grandfathering provisions which you are eligible for.

Or if you already have 15 years of qualifying experience, you don't need to worry about transcripts or the education requirement:  http://www.op.nysed.gov/cpalic.htm
