I got an email back from one of the NYS NASBA people.

Conclusion: Send both forms in (??????)

anmore, by "both" forms in, do they mean Form 1 and initial application, or initial application and registered program form (with transcripts)?  I would fight them on the Form 1.  Even if you qualify with 15 years experience or have met the 150 hour education requirement, it's another $377 fee to send in Form 1!

Each source says not to send any $$ in with Form 1 - the fee is still in the system as paid. I think I will do what I do with the tax dept - call again and see if I can get a consensus!

Oh cool, good to hear that at least everyone agrees that you don't need to include a fee with Form 1!  Although don't be surprised if you submit Form 1 with no fee and they kick it back to you, and you have to call them again to explain!  Geez, and I had thought I'd have it bad as a nontraditional student 10 years out of college who had to submit transcripts from 3 institutions.  Good luck with this!
