Yes, give your previous employer the Form 4b and instructions. You have to fill out the top part with your name, etc.
You can send in form 4 without the congratulatory letter. I am waiting for the congratulatory letter as well. From what I've read on this forum, we will probably get that letter in January or early Feb. But it's not needed to move forward with the licensure application.
