Nope. I gave to employer to fill out but because its holidays its going slow. And i am not sure if i can send out form 4 before 4b.

i don't know neither. I gave the form 4B to my previous employer, and i still hold the form 4.

You can send out Form 4 before your employer sends out Form 4b (or before you give 4b to your employer).  The order does not matter.

Well, the forms 4 and 4b don't say anything about needing to send out one before the other.  Although I mailed off form 4 and gave form 4b to my employers on the same day (11/23), and I think my current employer mailed off the 4b around 11/30.

From other New Yorkers' posts I've seen, it takes maybe 6-8 weeks to get the license, so hopefully I'll hear something in January or Feb.  FYI, you can look up your name at http://www.op.nysed.gov/opsearches.htm to see if you have an active license.
