It doesn't matter whether you work for them still. If you can, I would contact your former supervisor directly if they are a CPA and if not just request your boss to sign the form. In Maryland, you have to have the CPA mail the form and use their return address on the envelope.

sjfc07, have you received your license by now?  Did you send in Form 4 (application for licensure) at the same time that your employer(s) sent in Form 4B (the employer certification) or after?

CharliePapaAlfa, I'm curious about the award as well.  It sure would be nice to win anything that could help my resume.  From what I've heard, AICPA gives out the award around July of the next year.  Seems that at least one of the 2008 winners got the same total score of 390 that I got, so hopefully I'll have a shot next year at one of the 2009 awards.  I'll keep you all posted if I hear anything.
