Is there a form the CPA you have worked under fills out to verify your work experience or is just a simple letter stating that you worked under the supervision of a CPA from such and such date to such and such date? I am in Illinois if that matters. Thanks!

See the Verification of Employment/Experience form in the following packet of material:

I think you will find it at about page 15/18 on your Adobe.
