Crazy scores man, congrats.
Texas requirements seem to be much different, so with all due respect, don't listen to Gauri_3 on this.
All you do is fill out the Form 4, put your scores and work experience in the indicated spots, get a passport photo, get the thing notorized, and send it in. Keep in mind that you must account for all time from college graduation to the time you submit the form.
You don't do ANYTHING with Form 4B - you fill out your name at the top and give it to your employer. They will fill it out and send it in. It must be sent directly from your employer or else the education department will not accept the experience.
Simple as that.
No letterhead involved. It doesn't really matter if you use your company's envelopes or not. I sent mine in using my firm's envelope because that's what was handy at the time. It's just going to get thrown away - it's an envelope.
There is this thing called the code of conduct that guards against submitting false information. Basically, if the education department ever finds out you provided false information on any application form, POOF, there goes your license.
