So I rec'd my letter in the mail from the state of MD with my new license information, and my approval, I logged in and paid my dues. So my license is active now. Where does the certificate come from? Does that come from your state or from AICPA?

Not all state provide certificate and license. With certificate you cannot sign audit report; but license gives you the right to sign audit report. Those states are called "two-tier" state. License is all you need. In your case, you got the ultimate one.
