Oh yea. I resigned, and the partner threw me out of his office. Would not sign off on the form either. So I sent him a registered letter with the form asking for signature. Then 30 days later sent him a letter stating I would mail a letter to the board with all my paycheck stubs and tax return logs, and proove that I did the hours. If I did that, his license would be in jeaperdy.
Call your board. Tell them the storey and ask what you have to do to prove you did the hours, and what will happen to him for not signing the form. Then send him a threatening letter. If he doesn't budge, take him to the board.
This crap happens all the time. I agree this system sucks. I've heard so many stories about people getting screwed out of signatures.
