I am trying to apply for CPA license under Colorado board.

I just received the self study book for AICPA Ethics exam. I got an answer sheet along with the book, but i dont see any question paper as such. Am i supposed to take the questions from the web link given in the book or the 40 questions on the last pages of the book itself is the question paper?

Also, which one is the unique serial number i am supposed to enter? Is it the 5 digit number on left hand corner of the answer sheet or the 13 digit alpha-numeric code in the middle of the sheet?
