Hi everyone,

 

I passed the exam from NH and on the application form there is a section where it is required to list the parts of the exam and each date I sat for each part.  Should I put the all the exams that I sat (including the ones that I failed)? Or just the dates of the exams that I passed?

You should probably re-read the instructions.  My application (GA) has a form for the same thing but it's for NASBA to complete if I did not take the exams in GA.

No. This section needs to be completed by the candidate. See the link below for the form (Section 5). Maybe GA has a different form?

I also copied a picture of the related section below. Since there are more than 4 sections I assume they want you to write down all exams you sit (I am not sure why though) (including the failed ones?). But I wanted to be sure.

Include all your results just to be safe!! What could it hurt? Don't let such a minor factor stall your license!!
