So I passed my CPA exams 2 years ago and just finished my 2 year experience working under a CPA doing accounting work.  He filled in all the necessary information and has been notarized. 

Would I need to provide any further documentation? (It seems too easy to provide false information) and should I be the one to send it with my return address or the company's letter head?

^^ I am from Texas.  All I did was getting the form filled by my manager, a copy of her license (as she is not licensed in TX), and a copy of her resume showing her experience.  I got the Oath notarized.  I sent everything in an envelop.  I did not use Company's letterhead anywhere. 

As far as work experince goes, I don't think you need anything else.  We had other requirements to complete for some additional items here in TX - like the TX Professional COC exam and Ethics course for 4 credit hours and support for changes in educational qualifications, if any, since we filed for the exam.

Crazy scores man, congrats.

Texas requirements seem to be much different, so with all due respect, don't listen to Gauri_3 on this.

All you do is fill out the Form 4, put your scores and work experience in the indicated spots, get a passport photo, get the thing notorized, and send it in.  Keep in mind that you must account for all time from college graduation to the time you submit the form.

You don't do ANYTHING with Form 4B - you fill out your name at the top and give it to your employer.  They will fill it out and send it in.  It must be sent directly from your employer or else the education department will not accept the experience.

Simple as that.

No letterhead involved.  It doesn't really matter if you use your company's envelopes or not.  I sent mine in using my firm's envelope because that's what was handy at the time.  It's just going to get thrown away - it's an envelope.

There is this thing called the code of conduct that guards against submitting false information.  Basically, if the education department ever finds out you provided false information on any application form, POOF, there goes your license.

Good advice - don't listen to me.  From your posts, looks like you guys are very different from us here.

TX does not require 4Bs from all employers.  Any one employer where you worked for 2 years under a CPA is enough.  Additionally, TX does not require the employer to send the 4B directly to the board.  My supervisor filled it in, signed off and handed it to me.  If the supervisor has some concerns with the candidate's ethical/moral behavior or worthiness - they can write to the board separately.
