I sat for one part of the CPA exam back in 1989....then went into private, had a family, blah blah. Anyway, I am preparing to sit again. I am "grandfathered" under the 120 rule. I am confused as to what to submit in order to try and schedule the exam. Anyone with experience with this that can guide me?

You should contact your state co-ordinator at NASBA. They will be able to give you definitive guidance.

Alternatively, just call up and try to schedule another exam. Let them tell you what will it take to be reactivated. Please do share their response. Others may be able to learn from it.

Well, I finally got through to the state. I need to complete a Form 1 (Application)- but my fees were paid in 1989, so no additional money is necessary - and Form 2 (Certification  of Education). Paper files were purged in NYS in 2000; anyone inactive for 10+ years was purged. Electronic records remain and they were able to look me up by SSN.

ETA: I just finished searching the NASBA site and cannot locate where I find my state coordinator.  I just want to make sure NYS gave me all the information I need.
